Cervical radiculopathy, often called a pinched nerve in the neck, happens when a nerve root in the cervical spine becomes irritated or compressed as it exits the spine.
It’s a different problem from ordinary neck pain, since the irritation affects nerve function rather than just muscle or joint tissue.
Common underlying causes include a herniated or bulging cervical disc, bone spurs from cervical spondylosis, and narrowing of the nerve root openings from degenerative changes. The hallmark is pain, tingling, numbness, or weakness that radiates from the neck down into the shoulder, arm, or hand, whether from a single injury, years of gradual disc wear, or a sudden spike in strain on the neck. Because radiculopathy involves a compressed or irritated nerve root, treatment centers on intermittent cervical traction and neurodynamic (nerve gliding) exercises, which research shows can meaningfully reduce pain and disability when combined with manual therapy techniques and strengthening in patients who show clear signs of nerve root compression.
Joint mobilization and soft tissue work are added to reduce the muscle guarding that builds up around the irritated nerve, and a progressive strengthening program follows once symptoms start to settle.
Injections and pain medication can quiet a flare-up, but they don’t change the mechanical compression or irritation causing it.
Physical therapy works because it directly targets that compression, opening space around the nerve root through traction and restoring normal nerve mobility through neurodynamic exercise, while strengthening the muscles that support the cervical spine so the irritation is less likely to return once symptoms settle.
